Thank you for your interest in facilitating a Celebration Lifelong club. To propose a new club or schedule an existing club for Fall 2020, please complete the following information.
If you miss any fields after clicking the SUBMIT button, please follow “red” instructions to complete all required fields. When you successfully complete and submit your Club Proposal, you will see a message indicating that, plus you will receive an email with a copy of your Club Proposal.
